Background
Walnut allergy is primarily an IgE-mediated immediate hypersensitivity reaction. Walnut allergens identified using sera from walnut-allergic patients include Jug r 1, Jug r 2, Jug r 3, and Jug r 4. Among these, Jug r 1 is recognized by sera from more than 80% of walnut-allergic patients and is considered the major allergenic protein in walnuts.
IgE binds to Fc receptors on the surface of effector cells such as mast cells. Upon re-exposure to the allergen, cross-linking occurs between the allergen and IgE antibodies on the mast cell surface, triggering degranulation and the release of inflammatory mediators, which subsequently lead to clinical symptoms.
